This is the blank form for you to use in your defense of the eviction. Fill in the top of the first page with the proper information, then check the boxes you want to ask. Most are already selected for you. Fill out the proof of service on the last page and mail to the landlord's attorney [or to the landlord no attorney]. It is NOT filed in court. The responses are due 10 day after you mail it, which must also be at least 5 days prior to trial. If the landlord does not respond, you will need to file a motion to compel, which will require an attorney's participation, since it is not a simple form.
