It will happen sometimes that you lose the case for one reason or another, and there are errors by the judge, clerks, and otherwise which form the basis for your appeal. There are specific forms and procedures for filing the appeal, which begin with a Notice of Appeal. Thereafter, you need to file a request for the clerk's transcript of the papers you select which show the circumstances and basis for your appeal. Accompanying that is either a Statement of Appeal, where you provide a narrative of what happened at the critical hearing(s) upon whch you appeal, or in some cases where there is a court reporter, you ask for the reporter's transcript of what was said at those hearings.

You will provide the narrative in detailed and chronological fashion, and submit that in an email to cut and past into the court forms, or arrange to get the reporter's transcript for those days, if any.
